Exit Survey

Hello! We are JHSPH students currently taking a health survey course. As part of this class we have developed and are administering a survey on cell phones and distracted driving. You were selected to participate in this survey because you are a patron at the Hampton House Cafe. The survey will take up to 10 minutes to complete. Your answers provided here today will be confidential and anonymous. You are under no obligation to finish once you have begun and you can skip any question that you want. These data are being collected for class purposes so they will not be shared with anybody. At the end of the survey, we will give you a snack as compensation for your time. Would you like to participate in our survey?
