CONSENT TO PARTICIPATE IN RESEARCH
A research project on physical therapy residency selection is being conducted by Lisanne Meiners, PT, DPT, Megan Gallien, SPT, Natalie Hearn, SPT, and Casey Soileau, SPT in the Department of Physical Therapy at Louisiana State University Health Science Center - Shreveport.
The purpose of the study is to determine the factors influencing an individual's selection of a physical therapy residency program. With the information gathered, residency programs can modify/change their programs in order to attract future physical therapy residents.
You are being asked to take part in this study by completing an online questionnaire. Your participation will take approximately ten to fifteen minutes. Please be aware that you are not required to participate in this research and you may discontinue your participation at any time without penalty. You may also omit any items on the questionnaire you prefer not to answer.
There are no possible risks associated with participation in this study and your identity will remain anonymous at all times. If you should experience any distress, please be aware that you may contact Lisanne Meiners, PT, DPT, at (318) 813-2948 for assistance.
Your responses will be provided anonymously to protect your privacy. Potential benefits associated with the study include playing a role in the development and promotion of physical therapy residency programs across the United States.
