The purpose of this survey is to assess Provider's satisfaction with Laboratory services available to them. The survey will assess two areas: Clinic Laboratory Services and CRDAMC Main Laboratory Services. Thank you for your support.

The POC for this survey is CPT Luis Tejada, OIC of Ancillary Laboratory Services (ALS) at 553-7254 or Donna Potts and or Laura Sanfiorenzo (ALS Supervisors) at 288-8834/8223 respectively.
