My name is Carlos Hardy and I am a doctoral learner in the Harold Abel School of Social and Behavioral Sciences – Psychology at Capella University and an employee at The Georgia Department of Public Safety. Under the direction of Dr. Antonio Santonastasi in the Harold Abel School of Social and Behavioral Sciences – Psychology at Capella University, I am the primary investigator to research of the NEP Groups’ Mind Satisfaction Model (MSM) study on exploring the impact of I.Q. and general attitudes on job satisfaction in the public sector, from Headquarters, and I am inviting you to review the results of your MSM. The main purpose of this form is to provide information that may affect your decision about whether or not you want to participate in this research project and view results of your first surveys. If you choose to participate, please click accept at the end of this form to record your consent.
WHAT IS THE RESEARCH ABOUT? The research has two purposes: first, it contributes to the research of organizational behavior in the Industrial/Organizational Psychology field; and second, it seeks to contribute to the knowledge of scientific research by exploring a combination of surveys that analyze job satisfaction. A variety of the NEP Group past surveys have help organizations to understand employee in order to retain personnel and increase overall job satisfaction. This design should allow the research to assess the impact that, I.Q. and general attitudes have on job satisfaction, as well as provide the ability to examine the interaction of these three variables. The NEP Group has developed a unique combination of surveys, that when analyzed, help predict past and future job satisfaction. The results of your participation will provide additional insights of what role certain factors have on job satisfaction.
WHAT DOES PARTICIPATION IN THIS RESEARCH STUDY INVOLVE? If you decide to participate in this study, you will be asked to complete an informed consent form, by clicking to the appropriate key on the host site. Once the informed consent form has been accepted direct access to a series of four survey questionnaires will be granted. The four surveys consist of a demographic survey, a brief I.Q. quiz, job satisfaction, and general attitudes questions. To complete the entire questionnaire and skills package should take 15-20 minutes from start to finish. The primary investigator will forward anonymous data to appropriate raters for review. Each questionnaire will be interpreted, scored, and returned to the primary investigator for further analysis and group composition. Approximately, three days after the one-week deadline, instruction will be emailed to you on how to view results.
WHY ARE YOU BEING ASKED TO PARTICIPATE? You are being contacted to participate in this study because you are Peace Officer Standards Training (P.O.S.T.) certified, and have at least two years employment with The Department of Public Safety for the State of Georgia and are between the ages of 21-62. The information will allow a greater understanding of the best conditions to offer the greatest job satisfaction at their respective job, especially relating to law enforcement. Some officers were excluded from being asked to participate to avoid a conflict of interest with the primary investigator (i.e., Motor Carrier Compliance Division and officers serving in an administrative capacity).
WHAT ARE THE RISKS INVOLVED IN THIS STUDY? Although no study is completely risk-free, I do not anticipate any risks to you if you decide to participate in this study. There may be some questions that are uncomfortable to answer, please know that you will be free to skip these questions given in the research. If at anytime you decide to stop participation, you may do so without any penalty. This is your decision and your right.
ARE THERE ANY BENEFITS TO PARTICIPATION? We don’t expect any direct benefits to you from participation in this study.
HOW WILL THE RESEARCHER PROTECT PARTICIPANTS’ CONFIDENTIALITY? The results of the research study will be published, but your name or identity will not be revealed. In order to maintain the confidentiality of your records, all electronic data will be encrypted using Advanced Encryption Standard (AES) 256 a government data encryption grade standard and will be stored on the researchers password secured computer. The data collected through the online forms are stored in a MySQL database (an Open Source, relational database) which has been designed to ensure that all questionnaire and assessment data are stored separately from personal data in order to further increase the protection of participants' privacy. The data remains on the Questionpro database until the primary investigator exports the data from the database and compiles it into an Excel file. The final data file is then encrypted and password protected, and transferred to the primary investigators personal computer. After seven years from date of publication, the researcher will destroy the records.
WHAT HAPPENS IF A PARTICIPANT DOESN’T WANT TO CONTINUE IN THE STUDY? Participation in this study is voluntary. If you choose not to participate or if you choose to withdraw from the study, you may do so at any time. There will be no penalty. There is no oral or written obligation to continue the study if you choose not to participate. If you decide to withdraw from the study, your information will be removed from the record and all documents will be destroyed immediately. You will have until XX-2012 to withdraw from the research.
WILL IT COST ANYTHING TO PARTICIPATE IN THE STUDY? WILL I GET PAID TO PARTICIPATE? The assessment will be provided free of charge to the individuals wanting to participate. Participants will not receive any incentives for participation.
WILL PARTICIPANTS BE COMPENSATED FOR ILLNESS OR INJURY? You are not waiving any of your legal rights if you agree to participate in this study. However, no funds have been set aside to compensate you in the event of injury. If you suffer harm because you participated in this research project, you may contact the Capella Human Research Protections Office at 1-888-227-3552, extension 4716,Carlos Hardy at 347-746-0123, or Dr. Antonio Santonastasi at +1149 1633010443.
VOLUNTARY CONSENT I have read this form and I have had all my questions answered to my satisfaction. By going to the website and responding to the survey, I indicate my consent to take part in the study. Also, I will be asked to check the appropriate field on the survey site to record acceptance of informed consent to acknowledge (1) that you have read this form or have had it read to you and (2) that you understand this form, the research study, and its risks and benefits. The researcher will be available to answer any questions you have about the research. If you have any questions, please feel free to contact Carlos Hardy on 347-746-0123 or Dr. Antonio Santonastasi at +1149 1633010443.
This study has been approved by Capella University’s IRB 261218-3, effective from January 10, 2012 through January 10, 2013.

Your score results on the Mind Satisfaction Model (MSM) assessment show that you are highly conscientious about the quality of work and place you into the top 5 % on the MSM. Your performance depicts you as someone that can be depended on and follows directions well. You no doubt enjoyed doing this assessment so much that you may be an elite performer. People who score very high on this assessment are committed to their performing tasks to the point of gaining much of their personal skills. Such high performance on the assessment may mean that your performance levels are extremely high in other areas of cognition. You posses the right balance and you are truly committed to performing well at many tasks in life. To confirm these results, retesting of this assessment is needed. This assessment takes into account many factors that are not easily identified as a test taker. Initial results are only preliminary and should not be considered accurate or any indication on how you perform in your daily life. Accuracy will not be able to be established until retesting has been completed.
