Inclusive Performance Strategies will be presenting the final Strategic Plan to HIS staff and board during the January 18th board meeting. 

The plan has been compiled after significant input from staff and board, as well as focus groups and surveys with clients, community members, and partners. 

This survey is an opportunity for a final review of the objectives and key performance indicators for each domain. There is also a section to give final input on HIS's new Values statements. 

Please complete this survey no later than 12pm Wednedsay January 13th so that we can ensure any needed input is incorporated into the final plan. If you do not participate by that time, we will assume you have no additional input to give.

For your information, here are some definitions of the components included in the strategic plan: 

Strategic Priorities:  There are three strategic priorities for the HIS plan. These were designed in response to the "Big Questions" that came up during the planning process. All of the domains in the strategic plan are impacted by and help to acheive the strategic priorities. Each Strategic Priority has a measurement that will be regularly reviewed to ensure the organization stays mission-focused. 

Domains: These structure the plan and are defined by the Focus Areas.  They include Governance, Services, Communications & Engagement, and Resource Development. 

Objectives:  Each domain has an objective that serves as a 'big picture goal'. All of the key performance indicators for the domain align with this overall objective. 

Key Performance Indicators:  These indicate the desired state for HIS over the next 3 years. HIS will be successful in meeting the objective of the domain if the key performance indicators are met. Each year, action steps are drafted by the organization to acheive each key performance indicator. 

Action Steps: These are not included in this survey. Action steps are how the plan is operationalized. They are created by HIS executive staff with input from relevent staff and board members. Action steps are quarterly and are designed to work towards acheiving a given indicator.  Specific departments or people within the organization are responsible for completing the action steps each quarter. 
 
 
 
Strategic Priority Review 
 
 
The Strategic Priorities and their measurments are listed below. Please provide feedback accordingly. Within the final Strategic Plan, Inclusive Performance Strategies will provide a scorecard for tracking progress for each Strategic Priority. HIS will be responsible for creating benchmarks and annual goals for each Strategic Priority. 
 
 
Strategic Priority #1: HIS intends to prioritize service to the most needy and disadvantaged members of our community who are seeking medical and dental services.
Measurement: Ratio of ‘needy and disadvantaged’ patients as compared to overall patient population.

Strategic Priority #2: HIS will be open to and, as appropriate, pursue collaborative opportunities with health and human service organizations to create a more effective and efficient health care system.
Measurement: The number of collaborative opportunities presented and the number of collaborations actually established are documented, with rationale.

Strategic Priority #3: HIS will seek opportunities to provide services in socio-economically disadvantaged areas.
Measurement: Number of identified opportunities to serve in sites established as ‘disadvantaged’ based on research of community demographics and other available services.

• 1.1: HIS has a mission, values and vision statement that provides clear direction.
• 1.2: Board education and onboarding plan to support effective governance is implemented.
• 1.3: Structures are in place to systematically develop and review governing policies
• 1.4: Committees and advisory councils have defined roles and provide meaningful input for the Board’s decision making.
• 1.5: A committee process, including data review, is established to provide oversight of strategic priorities.
• 1.6: Roles and communications between board and management are clearly established and delineated

• 4.1: Fund Development plan that results in funding from a variety of sources is written annually & reported on quarterly.
• 4.2: Advancement Committee that supports fund development objectives and activities monitors the plan and activities regularly.
• 4.3: Guidelines and measurable goals for board involvement in fund development are established, maintained, and monitored.
• 4.4: Impact of Medicaid, self-pay, other insurances, and third party contracts on revenue is analyzed and reported on regularly
• 4.5: Human and technology resources needed to support fund development are secured.
• 4.6: The organization recruits, retains, and equips high quality, diverse staff.
• 4.7: A Volunteer Recruitment & Retention Plan is developed and monitored regularly.
