Team FSS:
You are invited to participate in our Safety Perception Survey. In this survey, everyone will be asked to complete a survey that asks questions about how you perceive FSS's safety programs and culture. The survey will take about 20 minutes to complete.
The information that is gained from the survey will be vital in aiding the Executive Management's development of FSS safety related goals of 2017. Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
