IGNITE Member Satisfaction Survey

Hello:

You are invited to participate in a survey for IGNITE. In this survey, approximately six to twelve people will be asked to complete a survey that asks questions about member satisfaction. It will take approximately 20 minutes to complete the questionnaire.

Your participation in this study is completely voluntary. Your honesty and candor are needed in order too make possible changes and improvements. It is very important for us to learn your opinions. Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. A focus group will be held on March 29, 2014 to discuss the results of the survey. Your feedback from the focus group will remain anonymous. The focus group will take approximately one to one and a half hours. Your attendance is greatly appreciated.
