Dear Respondent,
Mental and behavioral health care practiced in a primary care setting is a rapidly growing practice area. Unfortunately, the very swiftness of its growth has outpaced the research done in this field. Although many young professionals are entering the field, there are no readily available statistics on the requirements of working in the field, reimbursements, or the job satisfaction of those already working in primary care mental health service.
You were selected to participate in this study because you provide mental/behavioral health services in a primary care setting. With your participation, this study seeks to gather information and to provide professionals a better understanding of the work environment that primary care mental health providers experience.
This is a brief questionnaire that asks a number of questions about your training, qualifications, work environment, and general demographics. A measure of job satisfaction is also included.
I hope you will take a few minutes to complete this questionnaire. Without the help of people like you, research on the growing field of primary care mental health care could not be conducted. Your participation is voluntary and there is no penalty if you do not participate.
By answering the questions you are agreeing to participate in the study and allowing your answers to be used in a study investigating the demographics, job responsibilities, and job satisfaction of mental health providers in primary care settings. Your responses will not be identified with you personally, nor will anyone be able to determine which company you work for. Nothing you say on the questionnaire will in any way influence your present or future employment with your company.
