In an effort to evaluate and improve the effectiveness of LMHA's operations we would like each family to complete the following survey. We are looking for feedback on the organization as well as the coaching staff. The purpose of the coach evaluation is to provide feedback for LMHA’s Coach Selection Committee and the coaches. This tool will be used in the future selection of coaches, as well as a tool for coaches to see the areas where they excel and the areas that require improvement. It is encouraged to fill out a form for head coach and assistants as they all could be future candidates.

At the end of the survey, you will have the option of including your name and contact information. Individual surveys will not be shared with coaches. Parent survey will close on April 15th.
