Hello:
You are invited to participate in our Security Vulnerability Assessment of Ohio Valley Medical Center (OVMC) and East Ohio Regional Hospital (EORH). This survey is to help to identify and evaluate the hospitals current Security Operations and future Security needs. In this survey, a random selection of Hospital Employees will be asked to complete a survey that asks questions about current Security Operations and future Security needs. It will take approximately 5 minutes to complete the questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. Security of the Patients, Staff and Visitors should be everyone’s priority, so it is very important for us to learn your opinion. You are the eyes and ears of your respected hospitals and we value your comments.

This is an anonymous survey and your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
