Our Lady of the Lake Census 2012

We will be conducting a parish wide census. Our Lady of the Lake Parish has not updated the census information since 2005. We are overdue. This census will give us the chance to update parishioner information, and provide an opportunity to dialogue and renew ourselves with those who may have drifted away from the church for one reason or another.

For the purposes of this census form, the term “registered” is defined as having previously visited the parish office to provide your membership information. Your membership number is found on your collection envelopes.

Please check off one of the categories below. If you check one of the first three, please proceed to fill out the census form on the other side, and return it to the Parish Office; or you may place it in the collection basket. We will be calling the parishioners from whom we do not receive a form, so please complete the form, and return as soon as possible.

We are also looking for volunteers who would be interested in calling the families that do not turn in a census form. If you would like to help, please check the box below, and someone will be contacting you.
