CUB SCOUT PACK 684 END OF YEAR ASSESSMENT 2012-2013

Dear Parents,

You are invited to participate in our survey CUB SCOUT PACK 684 END OF YEAR ASSESSMENT 2012-2013. We ask all families participate in this assessment. We will use this assessment in our annual planning meeting to help guide any changes that would improve our Pack. Summary results will be provided to the Committee; not individual survey results. If any area cannot be evaluated, please leave it blank. It will take approximately 5-10 minutes to complete the questionnaire.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
