SDA Physicians,

Please take a few moments to complete this Physician Self-Evaluation. This self-evaluation is required and is designed to allow you to reflect on how you perform your duties & responsibilities as an anesthesiologist, member of our group, and as related to our Standards of Excellence for SDA physicians.

This self-evaluation is for your benefit. Thoughtfully evaluating yourself is a key to growth & improvement. Your individual responses are confidential and will not be shared with other physicians. The Physician Relations Committee will compile the results of all group members' responses. They will only review your self-evaluation and provide feedback and assistance at your request.


The self-evaluation should take about 10 minutes to complete. A copy of your completed evaluation will be emailed to you within 48-72 hours of completion by Woody Anderson. Please contact any member of the PRC (Bruce Baird, Kent Davis or Jon Erickson) if you have any questions.

Please complete this self-evaluation no later than November 28, 2008.
