Dear Military Commander:
You are invited to participate in DoD Sexual Assault Prevention and Response Office's Prevention Guide Leadership Feedback Survey. In this survey, approximately 50 Commanders will be asked to complete a survey that asks questions about how information is received and what prevention information/tools are most useful. It will take approximately 10 minutes to complete the questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. * What level of command are you currently filling or have most recently filled?
 
Tactical Command - This level of command has daily interaction with most military members assigned to a unit and most responsibility in prevention application and response to incidents (Typically Grades O4-O5)
 
Operational Command - Exercises command authority for implementation and have primary responsibility for resourcing (Typically Grades O6 - O9)
 
Installation Command - Exercises command authority to implement strategy over a geographic area that supports military units (Typically Grades O6-O8)
 
 
* As a Commander, what problems or challenges do you face with prevention implementation (Any social ill)? Is it more challenging when specifically addressing sexual assault prevention?
