Cover Letter with Implied Consent

Dear to whom it may concern:
I/We Lindsay Vik and Kelly Schultz are conducting a research project entitled "Project 7" as part of class research at South Dakota State University.
The purpose of the study is to research the correlation between involvement with community organizations and level of education and 2 demographic characteristics of Race and Gender.
You as a member of the community are invited to participate in the study by completing the attached survey. We realize that your time is valuable and have attempted to keep the requested information as brief and concise as possible. It will take you approximately 5 minutes of your time. Your participation in this project is voluntary. You may withdraw from the study at any time without consequence.
There are no known risks to you for participating in this study. While taking this survey if questions are of a sensitive nature, you have the option of not answering.
The benefits to you are you will be able to have a better understanding of your community. You will be able to see how being a part of a community organization affects the community as a whole. You will be able to understand who is a part of your community organizations.
Your responses are strictly confidential. When the data and analysis are presented, you will not be linked to the data by your name, title or any other identifying item.
Please assist us in our research and return the completed survey in the enclosed envelope.
