SURVEY INSTRUCTIONS

As you may know, there is a LEAN process improvement effort currently in the works surrounding DCAF. This improvement effort is focusing on improving the quality and content of a project’s deliverables as well the project team’s engagement with Shell’s TAs. These key areas are all crucial to the PCAP “control point” review process that we are all charged with as part of DCAF.

One way in which we’d like to measure our improvement is based on your judgment of various facets of this process, captured by this anonymous survey that should take no more than 5 minutes of your time.

A duplicate follow up survey will be sent out again well after the improvement measures have been implemented to help quantify our improvements.

Please provide accurate and honest feedback in regards to your involvement with projects being worked via DCAF ONLY through the Engineering Projects (EPr) group in UAD. This does NOT include any surveillance or P&T projects that are worked outside of the EPr group.

You must provide an answer for every question. If the question is "Not Applicable" or you are "Not Sure," please briefly explain why.

There will be a chance at the end of the survey to further provide your feedback in more detail. This will provide an avenue to voice any specific concerns that you would like to be addressed.

Thank you very much for your time and support towards this improvement effort!
