You have been a great team and I have really enjoyed working with you this year. Over the last week or so, I have had appraisal discussions with all of you and have tried my best to be objective while providing feedback to you.

I believe in a 2 way communication model where you should get an equal opportunity to provide feedbak to your manager (in this case me:-)). I have designed this short survey to capture your "genuine" feedback as some of you may hesitate to engage in this discussion in a F2F format.
