Dear Parents -
The Blessed Teresa of Calcutta CYO program is always looking for ways to improve. One critical area is to receive feedback about your child's sports program experience. At the end of each season, we ask all parents to take a few moments to reflect on how the sport your child participated in was received by both parent and child. We look for constructive comments about the sport, your child's coach, game format, etc. In completing the survey below, please keep the following points in mind:

* Coaches in our program are volunteers who sacrafice a lot of their own time for practices and games; they also undergo training required by the Archdiocese in coaching and working with young child. Their dedication is a great service to the parish.

* When providing feedback, please take the entire season into perspective, don't react to a single incident, it is important to look at the entire experience.

* Upon review of the feedback, you may be asked to sit with the CYO Athletic Director and/or Pastor to discuss any specific issues that they feel require further review.

* All comments/feedback will be kept confidential.
