You are invited to participate in this online questionnaire, which will assist in the development of DCT’s Network Architecture Map. This map will inventory the current network environment and infrastructure, and be used to construct and maintain a high performance network with the resilience to support the increased demands of future growth.

As a project team member and spokesperson for your department’s end-users, your input is critical to achieve this desired end state. Since the goal of this questionnaire is to gather honest feedback, respondents will not be asked to identify themselves by name. However, because some of the information sought is potentially identifiable, we cannot ensure complete anonymity.
