Hello: You are invited to participate in my survey about electronic data capture for condition surveys. The questionnaire will take approximately 10 minutes to complete and is based around 11 questions.

Your participation in this study is completely voluntary and you will remain anonymous. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions. Your survey responses will remain strictly confidential between the participant, the researcher and the markers. The data collected will be stored in a password protected file and will be destroyed after 6 months from the conclusion of the research. For the purposes of this questionnaire Computer literacy is defined as the knowledge and ability to utilize computers and related technology efficiently, it also refer to the comfort level someone has with using computer programs and other applications that are associated with computers.
