My name is Lieutenant John Kolarik and I am a student at the National Fire Academy in the Executive Fire Officer Program. You are invited to participate in a survey on Community Risk Reduction. In this survey, Chief Fire Officers or their delegates are being asked to complete a survey that asks questions about your specific Community Risk Reduction practices. The survey should take approximately 10 minutes to complete.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important to learn your insights.
