Dear Colleague,

As the Local Counter Fraud Specialist for NHS Surrey and I am currently assessing the level of counter fraud awareness of GP Practices working on behalf of NHS Surrey.

In order for me to determine the level of fraud awareness and possible weaknesses within Surrey I would be grateful if you could spare a few minutes to complete the survey.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.
