Dear GLIAC Athletic Directors:

You are invited to participate in this GLIAC sponsored survey regarding Team and Student-Athlete performance and Sports Psychology. In this survey, all GLIAC Athletic Directors are requested to answer questions pertaining to your individual thoughts about Sports Psychology. It will take approximately 10 minutes to complete the questionnaire.

The GLIAC office is exploring the potential of offering sports psychology resources to member schools in the conference as a pilot program beginning in the fall, however before we explore taking next steps we would like your opinions and thoughts.

Your participation in this study is completely voluntary, however your input is critical for survey accuracy. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your thoughts and opinions.

Your survey responses will be strictly confidential and data from this survey will be reported only in the aggregate.
