Good Shepherd Health System is gathering local data as part of developing a plan to improve health and quality of life in the community. Community input is essential to this process. You have been selected for a key informant survey because of your knowledge, insight, and familiarity with the community. The themes that emerge from these surveys will be summarized and made available to the public; however, individual survey responses will be kept strictly confidential.
