Purpose. The purpose of this survey is to gather your views on healthcare leaders serving as organizational teachers within Texas Health Resources.

Definition of terms. For the purpose of this study:

- A healthcare leader is defined as an employee who is responsible for leading divisional strategies and/or operations within the healthcare system.

- An organizational teacher is defined as an employee with expertise in a specific content area who shares such knowledge in a structured learning environment with other employees throughout the healthcare system.

- Attitude is defined as an individual’s personal evaluation toward a behavior.

- Organizational and professional norms are defined as influences and/or perceptions governed by the cultures of the organization and professional workgroups.

- Perceived behavioral control is defined as realistic constraints in self-efficacy, ability, means, and opportunity that may exist.

- Intention reflects a healthcare leader's willingness to serve as an “organizational teacher”, which includes an assumption that he/she possesses the motivation to perform the behavior under consideration.

Questionnaire. There are a total of 21 questions within six sections and should take respondents no more than 10 minutes to complete.

Scoring. There is no right or wrong answer as it represents your own personal perspectives and perceptions.

Confidentiality. By completing this questionnaire, you are providing your implicit consent to participate in this study. Your responses will be assigned a numerical identifier so that no one can match you to your responses.
