Hello:
You are invited to participate in an Evidence-Based Practice Project and educational session related to PEG Tubes. For this project, all nurses will be asked to complete a demographic questionnaire for general information, complete a pre-test, attend 2 educational sessions, and then complete a post-test. Each educational session will last approximately one hour.

Your participation in this project is completely voluntary but encouraged by administration as part of an educational series . There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you may skip the question or withdraw at any point. However, It is very important for me to learn your opinions and measure your knowledge after the teaching sessions have taken place.

Your responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. This information will not be shared with anyone. It will not be seen or shared with other nurses, administration or any hospital employee.
