How to take the OCAI survey

1. Respond to the survey while considering the “organization” as the Corporation as a whole.

2. The survey contains 12 questions. Under each question you will see 4 alternatives.

3. You have 100 points to divide among these alternatives, depending upon how closely each choice describes your organization. Each answer total must add up to 100 points. Do not use decimals.
