Hello, My name is James Rao and I am a student at The Institute for Clinical Social Work. I am also a social worker at the Hahnemann University Hospital Transplant Center. I am conducting a short survey about patient satisfaction at this doctor’s office.
You are invited to participate in our Patient Satisfaction survery. It will take approximately 10-15 minutes to complete the questionnaire.
Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point.
The purpose of the survey is to better learn about your satisfaction with regards to patient service at our office. Your answers will assist us in our ability to provide satisfactory service.
