Hello:

You are invited to participate in our survey relating to your own work performance and also that of your colleges. You personal performance review with a Practice Manager will be conducted between March and June 2013 and will involve a practice skills assessment, review of this survey and a strategic development plan for 2014-2015 .

In this survey you will be asked to complete questions about your attitude, ability, personality and other people you work with both directly and indirectly. The questions require written answers (no multiple choice) and it will take approximately 15 minutes to complete the questionnaire. Your participation in this study is very much appreciated and will help your Practice Manager complete appropriate assessment prior to your meeting with them. It is very important for us to learn your opinions and how we can improve on providing a safe, friendly work environment and to help you achieve a career path and/or job satisfaction.

Our company will continue to develop and change with new staff, new equipment, better systems and government legislation changes. We are committed to ensuring our employees are very satisfied in their current roles and that you receive quality training, excellent communication and interesting education.
