RESEARCH PROCEDURES
This research is being conducted to understand the role of physician websites in the search for health care providers. To participate, continue to the next page. The questionnaire will take around 20-60 minutes to complete.

RISKS
There are no foreseeable risks (physical, psychological and/or social) for participating in this study.

BENEFITS
There will not be any immediate benefits to you as a participant in this survey. Participation may help in furthering research in the area of health communication.

CONFIDENTIALITY
All personal information will be confidential and not disclosed in the paper, nor will it be shared with anybody.

PARTICIPATION
Participation is voluntary, and you may withdraw from the study at any time and for any reason. Participants must be 18 years or older to participate. If you decide not to participate or if you withdraw from the study, there is no penalty or loss of benefits to which you are otherwise entitled. There are no costs to you or any other party, nor is there any compensation.
