Dear Participant,

You have my sincere appreciation for agreeing to participate in my dissertation research. This study is about how individual members of a team make sense of their sometimes complex work in different ways. I hope that this may produce valuable insights for you and your team. This is an unprecedented opportunity to reveal often hidden aspects of your team work.

This study focuses on identifying patterns over time. To accomplish this, I am asking you to give 15 minutes of your time online twice a week for six weeks followed at the end by a 90-120 minute in-person group session to review the results of this research.

Unlike many researchers, I will not modify, categorize, or otherwise reinterpret your stories or reflections.  After six weeks we will convene a team/stakeholder "sensemaking" session where you all will look at the results and decide amongst yourselves what the patterns mean to you and how/why they may have formed.  I will simply record and later report on these findings as a case study.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project because your connection to your own survey responses will be strictly confidential.  All of your stories and answers are indexed to your own special three digit code known only to you and me.  If you feel uncomfortable with this process or in answering any questions, you can withdraw from the survey or the entire process at any point. If you have questions at any time about the survey or the procedures, you may contact me at XXX-XXX-XXXX or by email at [email protected]. I will add a final note of encouragement.  The more stories and accompanying questions you answer each week, the more data this will generate leading to stronger revelations and visualizations of possible patterns for us all.

Thank you very much for your time and support. You may start by inputing your unique three digit code in the box provided below and then clicking on the Continue button to begin the survey.
