I am Davia Nelson, a sixth form student of the prestigious high school of Immaculate Conception High School. My studies of Communication Studies requires me to issue questionnaires to a sample to complete a research project.

You are invited to participate in my questionnaire Impacts of Bleaching on High School students. In this survey, approximately sixty people will be asked to complete a questionnaire that asks questions about Skin Bleaching. It will take approximately ten minutes to complete the questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the questionnaire at any point. There are no attempts to offend the sample in any way. It is very important for us to learn your opinions.

Your responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
