We are conducting a survey for our Research Methods course at Vanier College. We would like you to take a few minutes to answer our questionnaire about general student health habits. This questionnaire should only take 5-10 minutes of your time.

Your responses are completely confidential. Your name or student number is not required to answer this questionnaire. Participation is completely voluntary. You may withdraw from the survey at any time and you may refuse to answer any questions. However, once your survey has been submitted, due to the anonymity of the survey your answers will not be able to be withdrawn. By responding to this survey you are providing your consent for your responses to be used in our project.

The purpose of this survey is to help us understand how students live. We are looking at specific activities/behaviors because we would like to see if there are relationships with regards to healthy behavior and organization.
Please answer truthfully and to the best of your knowledge. There are no right or wrong answers, we are only looking for opinions.
