This survey is designed to evaluate the Teach 21 program and ascertain how much your classroom productivity has increased as a result of your participation in the Teach 21 program. Data gathered from this type of survey will provide feedback to evaluate the program, indicate if the program assisted teachers to engage students and be more productive, ascertain the enduring effect of the instruction and equipment provided by the program, determine in dollars the district’s return on its investment into the program, and meet requirements for my dissertation. The survey may take 25 minutes to complete and you may log out and back in to complete the survey. Please answer all questions. 
The findings will be published in a doctoral dissertation through Nova Southeastern University.
