Dear Sir/Madam/Miss,
I am inviting you to participate in a research study titled ‘The relationship between self-efficacy and burnout among teachers teaching students with Autism’. This study is being conducted for my dissertation research at University of Malaya.
This survey consists of 3 parts which are demographic information, teacher self-efficacy belief and teacher burnout. The survey should take about 10-15 minutes to complete. Please read each statement carefully and follow the instructions as stated. Be sure that you do not circle or tick more than one answer for each statement. To change an answer, draw an X through the incorrect response and circle/tick the desired response. Surveys should be returned in order to be used the data in the research study.
