Online Study Informed Consent Form

DESCRIPTION:
The purpose of this study is to establish national best practices for the delivery of Two-way DL programs by determining the common components among the most successful Two-Way DL programs across the nation. Participants will be asked to complete a survey that asks questions about the most crucial components of their school's Two-Way Dual Language Program. Approximately 10 minutes are required to complete the questionnaire in a single session.

EXCLUSION CRITERIA:
Participants must be at least 18 years of age and a bilingual program director at an elementary school that employs the two-way dual language model in order to participate in this study.

RISKS:
There are no foreseeable risks associated with this project.

BENEFITS:
a) You will have an opportunity to contribute to the establishment of national best practices for two-way dual language program delivery by participating in this research, which may further the cause of additive bilingualism.
b) When your participation is complete, you will be given an opportunity to receive the results of this research at its conclusion, which may prove beneficial to your school for the purpose of program expansion and professional development.

CONFIDENTIALITY:
No identifying information will be collected from you, including your IP address, so no one will be able to distinguish your questionnaire from any other respondent’s questionnaire. Our interest is not in the individual responses but rather the aggregate data. The data derived from this study could be used in reports, presentations, and publications but your will never be individually identified.
