You are invited to participate in our survey - Morale - Establishing and Creating a Positive Work Environment. In this survey, all the staff members of 4 South will be asked to complete a survey that asks questions about what morale means to you and ideas and suggestions on how we can create an improved work environment. It will take approximately 5 minutes to complete the questionnaire.

Your participation in this survey is completely voluntary, anonymous and strictly confidential. Your partcipation and honestly is strongly encouraged to help us in improving the work environment and the place that you spend a good deal of your life. Your opinion matters and your ideas and perceptions will assist in creating a better work environment. Data from this survey will be reported only in the aggregate. Your information will be coded and will remain confidential.
